DUTIES:
This position is located in Florence, AL 422DD Cox Blvd Sheffiled, AL 35660.

The incumbent serves as the RN Care Coordinator for the Florence-Shoals CBOC he/ she is selected. The RNs are assigned to a Patient Aligned Care Team (PACT) Model. The Nurse will be responsible and accountable for coordinating patient care for individual and groups of patients ranging from the young adult, age 18, to the elderly adult, age 65 or older. This Nurse is clinically and administratively responsible to Primary Care Nurse Management. The care coordinator uses the nursing process and evidence-based practice to collaborate with the teamlet (patient, primary care provider, RN care coordinator, and clinical support technician and lager teams (family/ caregiver, internal and community based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patientÂ's need. The incumbent provides patient and family health education with a focus on self-management, prevention, and wellness based on the patientÂ's goals. The RN demonstrates leadership by serving as an advocate for patients, team player to colleagues as he/she continues to enhance his/her own and the teamÂ's professional growth, development, and practice. The RN performs independently in collaboration with the provider to carry out the plan of care for the veteran. He/ she appropriately delegates to other team members but is responsible for providing case management and care coordination activities, clinical triage and making more complex clinical decisions. The RN uses face to face visit, telephone visits, secure messaging, and possible inpatient visits to patients assigned to the teamlet. The RN provides back-up for other teamlet RN staff. The RN tracks patient outcomes to determine if interventions are effective. The RN must be able to complete tasks assigned to the LPN but the majority of the role encompasses performing nursing activities that include the core of the nursing process (assessment, diagnosis, outcome identification, planning and evaluations) and that requires specialized nursing knowledge, judgment and/ or skill. The incumbent provides direct nursing care to individual and groups of patients and assists with the direction and supervision of other assigned clinical and/ or clerical staff.

QUALIFICATIONS REQUIRED:
Citizenship: Citizen of the United States (Noncitizens may be appointed when it is not possible to recruit qualified citizens per VA Handbook 5005/2, Chapter 3, Section A, paragraph 3g).

Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant. The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

Licensure : Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

English Language Proficiency: Must be proficient in spoken and written English.

Physical Standards: Satisfactory physical examination as determined by the Employee Health Unit upon appointment only. See VA Directive and Handbook 5019 ( http://vaww1.va.gov/ohrm/HRLibrary/Dir-Policy.htm ). Functional requirements include light lifting under 15 lbs, light carrying 15-44 lbs, walking and standing and standing for up to 8 hours, use of rapid mental and muscular coordination simultaneously, ability to read type written characters (glasses permitted), ability to hear the whispered voice (hearing aid permitted), repeated bending for up to 8 hours, etc.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research.

Education/Experience Requirements:

  • Nurse I, level I-Associate Degree or diploma in nursing; no experience
  • Nurse I, level 2-Associate Degree or Diploma in Nursing, approx. 1 year experience OR Associate Degree in Nursing and a Bachelors in a related field, no experience, OR BSN, no experience
  • Nurse I, Level 3-Associate Degree or Diploma in Nursing, approx. 2-3 years experience OR Associates Degree or Diploma in Nursing and Bachelors degree in a related field, approx. 1-2 years experience OR BSN, approx. 1-2 years experience OR MSN or related field with a BSN or BachelorÂ's degree in related field, no experience.
  • Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
Enhanced Qualifications:
  • Two (2) years of successful general medical-surgical practice experience preferred.
  • Two (2) years of Ambulatory Care/ Case Management experience preferred.
  • BSN or higher nursing degree preferred
  • Current BCLS certification
  • Excellent interpersonal and communication skills (oral and written).
  • Demonstrated ability to assess and prioritize multiple medical and surgical diagnoses.
  • Demonstrated excellent physical assessment skills.
  • Demonstrated ability to be dependable, self-directed, and flexible, including flexibility in assignment and time scheduling.
  • Demonstrates leadership skills.
