As a Hospice Registered Nurse , you will: Provide care and emotional support to the actively dying patient. Assist with activities of daily living to provide comfort for patient. Complete initial history and physical assessment. Assess individual status and care / instruction needed by the patient. Prepare and implement patient’s plan of care while collaborating with Hospice Team. Provide hands-on care, case management and evaluation of the care plan, and education of the patient utilizing Gentiva Care Notes. Revise plan in consultation with physician based on ongoing assessments.
In addition, a Gentiva Hospice Registered Nurse : Has the autonomy to make individualized patient-centered decisions for optimal clinical care. Works with other highly skilled clinicians through a multi- or interdisciplinary team approach. Qualifications
- Diploma, Associate or Bachelor Degree in Nursing
- A minimum of two years nursing experience preferred
- Strong med surg, ICU, ER, oncology or acute experience
- Hospice experience a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Good organizational and communication skills
- Ability to partner and collaborate with Hospice Clinical Team
