DUTIES
Registered Nurse Case Manager Position: * You will act as the RN Case Manager for Hospice Patients. * The Hospice RN Case Manager is responsible for the planning, coordination, provision and documentation of care for terminally ill patients from day of admission through day of discharge. * Works in compliance with federal and state regulations, reimbursement guidelines as well as the organization's policies and procedures. * Assures quality care by reviewing and collaborating with the multidisciplinary team including RN, Social Worker, C.N.A., Chaplain and Volunteers in conjunction with the Patient Care Coordinator, RN. * The Hospice Nurse participates in quality control programs. * The position reports to the Patient Care Coordinator, RN. * Works with IDT to develop and implement hospice plan of care. * Supervises Hospice Aides and LPNs. Job Specifications: * Registered Nurse with a current Alabama nursing license. * Knowledge of or willingness to learn hospice care. * Minimum one year nursing practice or equivalent experience required. * Hospice, Oncology, or Home Health and/or long-term care preferred.
