Adheres to all Brookwood Home Health policies and procedures and maintains standards of practice and documentation that are consistent wi! th State and Medicare regulations and accreditation standards.
2. Upon acceptance of a patient, follows through with all visits ordered throughout patient episode. Reports to the office as per policy, completes paperwork, and gives Director of Nursing updates on patients.
3. Provides to the RN, a report regarding the patient's current physical and emotional status, any change in the patient's condition, visit schedule, required care and any noted need to change to the Plan of Care, at a minimum of weekly, and as deemed necessary by a change in the patient's condition.
4. Participates in case conference/coordination of care communication.
5. Participates case conferences/coordination, care communication, orientation, in-service meetings, and other training sessions as directed by the Director of Nursing.
Current LPN license in the state of AL; Must have CPR certification. Minimum of 1 year RN Psych. experience required. Knowledge ! of Medicare coverage criteria for therapy services to Home Car! e Patients. Ability to audit charts for documentation, QI and UR purposes. Current driver's license, reliable transportation, and auto liability insurance.
