Duties:
The Telehealth Nurse is a self-directed professional who uses the nursing process and evidence-based practice ! to provide patient-driven, holistic care with the collaboration of the Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support Technician) and larger Team (family/caregiver and internal and community-based services and disciplines involved in providing care to the patient). The Telehealth Nurse is a member of the expanded team. She/he utilizes health information and telecommunication to promote therapeutic relationships and shared decision-making between the patient and interdisciplinary team across the continuum of care to support patient self-management, prevention and wellness. She/he is a member of the Care Coordination Home Telehealth (CCHT) team. Meets the needs of customers while supporting VA missions. Consistently communicates and treats customers (Veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Provides the customer with consistent information. Coordinates care and home care plannin! g with the patientÂ's core team. Assists with the navigation o! f health care systems and intervenes or refers as needed to facilitate the achievement of patient outcomes. Assesses patients for appropriate use of Telehealth devices; follows procedures for ordering through Prosthetics, documents installation and education in CPRS. Integrates CCHT data into assessment, planning, intervention, evaluation, and revision of life care plan with input from the patient, family, core and expanded team. Appropriately documents interventions using approved CCHT coding and reviews appropriate patient documentation to determine the need to update the patientÂ's life plan of care. Guides others in the recognition of and participates in the resolution of ethical issues. Demonstrates sensitivity to the cultural values and beliefs of patients and staff. Manages the placement of available CCHT equipment for the best utilization by patients enrolled in the care coordination panel. Manages the inventory of CCHT equipment. Serves as a resource and advocate i! n the protection of patient and family rights.

The Central Alabama Veteran Health Care System (CAVHCS) is a consolidated Health Care system that includes VA Medical Centers in Montgomery and Tuskegee Alabama as well as Community Based Outpatient Clinics (CBOC) in Dothan Alabama, Monroeville Al, Ft. Rucker Alabama and Columbus Georgia. The Primary Duty Station for this position is Montgomery, Alabama. The incumbent may be required to work on site at the other locations as required.

NOTE: Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and step of the applicant selected.

Tour of Duty: 8:00 AM - 4:30 PM Central Time Monday thru Friday; however, employee may be required to work extended clinic hours and/or rotate work shifts. Weekend coverage or extended clinic hours may be required.

Qualifications & Evaluations

BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Full unrestricted Licensure, Certification, or Registration required
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate N! ursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

EDUCATION/EXPERIENCE REQUIREMENTS:
The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qu! alifies for professional nursing registration constitutes the completio! n of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

EDUCATION/EXPERIENCE REQUIREMENTS:
Nurse II - Bachelor of Science in Nursing (BSN) and 2-3 years experience; OR
Associate Degree or Diploma in Nursing AND Bachelor's Degree in a related field AND 2-3 years experience; OR
Master's Degree in Nursing or related field AND BSN or Bachelor's Degree in a related field AND 1-2 years experience (If MSN obtained in a Bridge Program, no BSN required); OR
Doctoral Degree in Nursing and no experience
Nurse III - Master's Degree in Nursing or related field with BSN or Bachelor! 's Degree in a related field AND 2-3 years experience; OR
Doctoral Degree in Nursing or related field AND 2-3 year experience.
(If MSN obtained in a Bridge Program, no BSN required).

Preferred Experience: Bachelor of Science Degree in Nursing. GRADE DETERMINATIONS:
Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

The criteria are as follows:
Practice: practice, ethics and resource utilization;
Professional Development: performance, education/career development;
Collaboration; collaboration, collegiality;
Scientific Inquiry: quality of care, research.
