Responsible for a variety of activities including utilization review, discharge planning, care coordination and clinical resource management for assigned patient populations. Interacts with Medical, Nursing and Ancillary department staffs to facilitate quality based, cost-effective patient outcomes. Works with the medical staff as well as the multidisciplinary team to ensure that patient care is provided in the most appropriate setting to ensure the wise utilization of resources. Applies and documents evidence-based screening criteria to all patient admissions and continued hospital stays to ensure appropriateness of admission and medical necessity for services.
Maintains working knowledge of third party payer and regulatory agency requirements. Develops, communicates, implements and documents an age appropriate post-hospital plan of care for patients requiring post-hospital services. Delegates the execution and coordination of the post-hospital services that require clinical expertise. Analyzes aggregate trends and integrates findings into continuous quality improvement (CQI) and/or healthcare management standards to accomplish clinical process improvement.
Typically, the hours of operation are business hours, but the position may require work at times other than normal business hours in order to meet the needs of the patient, family and/or department. Weekend on-call coverage may be assigned as needed to meet various patient needs. Responds to the unique needs of all patients ages newborn through geriatric. Minimum Qualifications Bachelor's degree in Nursing and five (5) years of related experience required.
Work experience may NOT substitute for education requirement. Posting Salary Minimum 53144.00 Posting Salary Maximum 82805.00 Employment Status Full Time Benefit Eligible Shifts Day / 1st Shift Licensure / Certifications Currently licensed or licensed by endorsement as a Registered Nurse (RN) by the Alabama Board of Nursing required.
