We are seeking a knowledgeable, compassionate RN to provide direct care to hospice patients in an inpatient unit within the hospital. As the Hospital Inpatient Liaison, you will network with local hospitals, nursing homes, assisted living facilities and the surrounding community when needed to sign on additional hospice patients. Our Inpatient Liaison will also conduct an initial evaluation of the potential hospice patient to determine admission appropriateness. As an Inpatient Liaison, you will also partner with our internal marketing and sales teams and will act as the clinical expert for our inpatient program. You will also handle discharge planning for patients being transitioned into an outpatient setting and will also meet with families to discuss services and coordinate appropriately. This position offers p! otential for growth, a friendly staff, and an opportunity to i! nteract with patients and their families on a more personal level.

Responsibilities for the Hospital Inpatient Nurse Liaison include:

Conducting visits at hospital for inpatient hospice patients
Partnering with internal marketing and sales teams
Acting as the clinical expert for our inpatient program
Coordinating, assessing, and implementing patients' care
Providing accurate and timely documentation of patient services to reflect plans of care
Network with hospitals, nursing homes, assisted living facilities and surrounding community
Discharge planning, coordinating admissions, referrals and meeting with families to discuss services.
Attending weekly interdisciplinary meetings
Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials Qualifications:

RN licensure (current, ! unrestricted) in the state location of position
3 years of clinical experience is required
Hospice experience is required

Additional Assets Preferred:
Excellent communication skills
Experience in long-term care is a plus
Previous Hospice Liaison experience
Computer proficiency is a must
Background in home care and/or hospice care is highly preferred
Hospital experience is highly preferred
