Delta Team RN - Registered Nurse needed to work with two nursing facilities in northern Alabama that are participants in the Nursing Facility Initiative. RN must live within an hour’s drive of each facility (Decatur and Fa lkville). The RN will work hand- in- hand with the nursing facility to reduce avoidable hospitalizations. They will work with a variety of stakeholders (facility housekeeping and dietary staff, CNAs, nursing staff, medical staff and local hospital staff) and the Certified Registered Nurse Practitioner (CRNP) or physician to advance these goals. The RNs’ primary goals are to improve the overall management of the Population of Focus residents in the facility, improve clinical outcomes, and lower the costs associated with caring for these long-term care residents. Current State of Alabama unrestricted registered nurse licensure required. Long-term care, subacute, LTAC, hospital, home health and/or hospice experience required and working knowledge of long-term care quality indicators and quality measures preferred. Competence, experience and knowledge in conducting educational programs preferred. Must have strong nursing assessment, judgment and critical thinking skills. Good organizational skills with the ability to foster teamwork, multi-task, and adapt quickly to change. Ability to work with patients/families of all ages, with diverse physical conditions and social/cultural backgrounds. Excellent written/verbal communication and listening skills. Proficient in Microsoft Office Suite including Power Point required. EOE/Minorities/Females/Vet/Disabled
