Direct Care Staff provide personal care to individuals with intellectual disabi! lities in a residential setting.
Duties include but are! not limited to: assisting residents with personal hygiene, daily living activities,
documenting behavior and assisting with medications. ALL applicants must present a high school diploma/GED, Valid
Driver's License, proof of valid auto insurance and successfully complete the E-Verify process. All Applicants MUST
PASS a drug test, background check, TB test and have a clean driving record. Benefits available for full time (40hr)
employees after 90 days.
Summary
As part of the Ability Plus Health Services Team, the Licensed Practical Nurse (LPN) will assist the Registered Nurse (RN) in the coordination of patient care activities. The LPN will support the implementation of the program
processes as required of people with intellectual disabilities and other developmental disabilities.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
. Perform skilled nursing services under the super! vision of a staff RN
. Observes, records and reports to the supervisor on the general physical and mental conditions of the patient; reports pertinent changes
. Administers prescribed medications and treatments
. Assists the RN in performing specialized procedures
. Assists the patients with ADLs
. Perform nursing duties within the standard scope of practice
. Responsible for reports, record keeping and progress notes
. Receives and follows physician orders within the realm of practice for an LPN and within the standards of home health practice, but may not alter the plan of care.
. Participates in nursing report
. Participates in QA/QI plan and process
. Responsible for adhering to all practice standards as they apply to patient care
EDUCATION/BACKGROUND/EXPERIENCE:
. Current LPN License
. Minimum experience of 2 years as an LPN
. Experience in wo! rking with people with intellectual disabilities
. Ability to ! be on call 24/7
. High School Diploma or GED
. Current Driver's License and vehicle insurance
. Acceptable Background Check & DMV Report
. Social Security Card
PHYSICAL/MENTAL DEMANDS AND WORKING CONDITIONS:
. Working conditions are normal for a home health care environment.
. Work will require local driving as needed.
. Work requires ongoing interaction with intellectually disabled clients.
. Work can require frequent walking, standing, bending and lifting up to the weight of an adult.
