This is a Part-time position to manage the plan of care for a caseload of patients, assuming 24-hour responsibility for patient caseload. To serve as the liaison with the multidisciplinary care team. To plan care management as achieved through coordination of care with physicians, nursing staff and other health professionals to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources. May coordinate patient follow-up clinics and schedule tests and surgeries.
This Part-time position will also provide direct and/or indirect patient care in a research or clinical environment This could include coaching and educating patients to assisting with coordinating transitions of care. May coordinate clinic research, perform clinical tasks and/or provide for patient and family educational needs. The telehealth program will have aspects of research in that the things we learn from our implementation will be used to help the field of palliative care advance in this method of palliative care dissemination. Develops a relationship with the patient as an integral member of the team. Utilizes knowledge of patien t’s age and cultural diversity into the provision of patient care. Provides telephone advice per protocol. Conducts needs assessment and develops an individualized treatment care plan. Participates in regular team meetings and peer review activities.
(Hourly Rate: $26.57 - $34.81)
