This position assists in the provision of nursing care to the home health patient and provides skilled nursing care to adult patients, on an intermittent basis in the place of! residence according to the physician’s plan of care and under the supervision of a Registered Nurse. Maintains a current medical record for each assigned patient, containing elements required by Federal and State regulations and Agency policy. Maintains expected productivity standards. Maintains liaison and collaborates with physicians and other health care personnel to facilitate continuous care and coordination of services. Maintains standards of professional nursing practice. Participates in agency functions as required or requested. The person in this position works under general supervision, is responsible for various shifts, may be subject to over 40 hours per week and/or callback as required, and may also be required to remain on campus immediately before, during, and after severe weather and/or disasters.
QUALIFICATIONS FOR JOB:
- Licensed LPN in the state of operation.
- Graduate of an accredited school of nursing.
- Poss! ess a minimum of three years of nursing with one year home hea! lth or community health experience. Medical-Surgical experience may be considered.
- CPR certification
- Must possess a sound knowledge of nursing principles, procedures and elements of patient teaching.
- Must understand Medicare regulations, Conditions of Participation, credentialing criteria/standards and PPS compliance.
- Must be able to communicate articulately and comprehend written and verbal communications.
- Must be able to function efficiently in a stressful environment.
- Demonstrate good interpersonal relationship skills, works in a team responsibly and independently in the home care setting.
- Valid state Driver’s License and current auto insurance verification.
- High tech skills such as: Phlebotomy trained, Hyperbariac wound care and Wound Vac certification are strongly recommended.
